    Register now for the Summit Cheer Clinic. This instructional clinic is open to students in grades 1-5 and will be held January 29, 6:00-7:30pm, in the Rose Campus gymnasium.
  • PERFORMANCE: Participants will be featured at halftime during the Friday, January 30, varsity boys basketball game. 

    WHAT'S INCLUDED: Clinic fees include 90 minutes of instruction, a custom cheer t-shirt, a bow, and a snack.

    WHAT TO BRING: Please were athletic clothes and tennis shoes.

    REGISTRATION DEADLINE: Monday, January 19, 2024 (to guarantee a t-shirt)

    FEES AND PAYMENT: $45, payable at the time of registration.

  • I, the parent listed above and signing below, hereby grant permission for my child, submitted on this form, to participate in cheer activities organized by Summit Christian Academy (SCA). I understand and agree to the terms and conditions outlined in this waiver. 

    Assumption of Risk: I am aware that upper-level cheer activities involve physical exertion, including but not limited to jumping, stunting, tumbling, and choreography. I acknowledge that there are inherent risks associated with these activities, including the possibility of injury. I understand that SCA will take reasonable precautions to ensure the safety of participants, but accidents may still occur. Elementary cheer activities are limited to jumping, low-level stunting, and choreography.

    Health and Medical Considerations: I affirm that my child is physically and mentally capable of participating in cheer activities. I have or will disclose any known medical conditions, allergies, or disabilities that may affect their ability to engage in these activities. I understand that it is my responsibility to inform SCA promptly of any changes in my child's health status that may impact their participation.

    Code of Conduct: I understand that SCA has established a code of conduct for participants, which includes expectations for behavior, sportsmanship, and adherence to the rules and guidelines set by the coaches and staff. I agree to ensure that my child abides by this code of conduct and understands the consequences of any violations.

    Release and Indemnification: I release, discharge, and hold harmless The Assembly at Broken Arrow dba Summit Christian Academy, its employees, agents, volunteers, and representatives from any liability, claims, demands, or causes of action arising out of or related to my child's participation in cheer activities. I agree to indemnify The Assembly/SCA for any damages or losses incurred as a result of my child's actions or behaviors during these activities.

    Media Release: I grant The Assembly/SCA permission to use photographs, videos, or other media recordings of my child taken during cheer activities for promotional or educational purposes, including but not limited to the school's website, social media platforms, and marketing materials.

    Emergency Medical Treatment: In the event of an emergency where immediate medical attention is required, I authorize SCA staff to seek medical treatment for my child and to make decisions regarding their health and well-being. I understand that SCA will make every effort to contact me or the emergency contacts listed in my child's records.

    By signing below, I acknowledge that I have read and understood this Permission to Participate Waiver. I agree to the terms and conditions outlined herein and voluntarily grant permission for my child to participate in cheer activities at Summit Christian Academy.
